Our SFC program allows a family member who lives with an elderly or physically disabled, MEDICAID ACTIVE, OR MEDICAID ELIGIBLE recipient to receive a weekly, tax-free stipend for providing daily assistance and supervision.

Our PSS program allows a family member or friend, who is employed and may or may not reside with a MEDICAID ACTIVE, or MEDICAID ELIGIBLE care recipient, to become a Lavender Meadows W-2 employee. As a PSS provider, you will receive a weekly salary for in-home support services.

Live in Georgia in one of these counties: Cherokee, Clayton, Cobb, Dekalb, Douglas, Fayette, Fulton, Gwinnett, Henry, Rockdale, Burke, Columbia, Richmond, Forsyth, Hall, Newton, Bartow, Haralson, Paulding, Polk, Butts, Carroll, Coweta, Heard, Lamar, Meriwether, Pike, Spalding, Troupe and Upson
Meet Nursing Home Level of Care (need help with day-to-day activities)
Are able to direct your own care/services (if not, have an individual representative to appoint)
Are at least 21 years of age with a recognized physical disability
